Endora, una sperduta cittadina dell'Iowa, popolazione 1091 anime. Gilbert è un giovane uomo, poco più che un ragazzo. Ha una madre il cui peso, oltre 250 chili, sta lentamente distruggendo le fondamenta della casa. Non esce di casa da anni e vive mangiando, fumando e guardando la televisione in continuazione, tanto da essere diventata una sorta di mostruosa attrazione per i ragazzini che vengono a spiarla dalla finestra. Il fratello minore di Gilbert è handicappato. Incapace di badare a se stesso, ma capacissimo di mettersi nei guai, è sorvegliato e accudito amorevolmente da Gilbert. Un giorno arriva a Endora una sconosciuta che scatena una serie di eventi inaspettati.
